The Sunday School and Nursery Programs are a nut free zone. We will not be offering any snack that may contain nuts and we ask that you do not send your child with a snack that may contain nuts. We do have a garden outside in the play area that children may access during play time, if you think your child could be allergic please let the Sunday School and Nursery know at drop off.
Any adult listed on this form will be given permission to pick up your child. Please list your information and any other guardians of your child that have permission for pick up and contact.
Please provide an emergency contact of an adult that is not listed above. This emergency contact will also have permission to pick up a child.
Please list the names of approved adults for pick up. Please write their legal name as listed on ID, as adults who are new to volunteers/staff will be asked to show ID.

In an effort to be good stewards of the health of all children and church members we have adopted the following Health Policy for our Children/Youth Programs.
If Your Child Shows Symptoms of Illness During a Children/Youth Program:
If symptoms of illness are observed or reported, we will ask you to return home with your child. If your child shows any sign of illness: a fever of 100 degrees or above, shortness of breath, excessive coughing or drainage from the nose, undiagnosed or unexplained rashes or skin sores, as well as vomit, or diarrhea we will contact the guardian who dropped them off and inform them that they need to pick up their child. If the drop off adult cannot be contacted we will contact an adult listed on the emergency contact list or pick up list, and have them come get the child.
If Your Child Shows Symptoms of Illness Before Sunday School/Nursery Care:
Children MUST stay home when they are sick with symptoms of respiratory illness such as fever and cough and any of the symptoms listed above.
Our sick policy states that children must be fever, vomit, & diarrhea free for 24 hours before attending any Children/Youth Programs. Public health advises staying home for 48 hours after the last vomiting episode. If a child has persistent lingering symptoms a clearance note from a doctor may be required
Emergency Medical Consent
Elk Grove United Methodist Staff & Volunteers have my permission to obtain emergency medical treatment for my child/children when I cannot be reached or if a delay in reaching me, it would be dangerous for my child. I understand that I assume all financial responsibility for any treatment or injuries sustained by my child while they are in child care.
This provides us permssion to photographed or video record your child during church activites. My child's image may appear in print or online (website/social media) promoting the church's activities.
I undestand that my child's name will not be used to identify my child. If I would like to withdraw my permission, I may do so at anytime.
